HOW TO MAKE AN APPOINTMENT

Consultations and treatment are by appointment.

The reception is open from 8.45am until 12.45pm and again from 1.45pm until 5.00pm every weekday.

Additionally a receptionist is on duty until 6.00pm on Tuesday evenings and from 8.15 on Friday mornings.

If you are unable to attend an appointment please give reception as much notice as possible so that another patient may benefit from the space created.

If you have an emergency outside of normal surgery hours then please ring the practice number (01237 472159) and details of emergency cover will be given by recorded message. Please do not leave a message on the answer machine as these are not recorded.

Please note:- A charge may be made for an emergency recall to open the surgery out of working hours.

YOUR MEDICAL HISTORY
When you first attend the practice as a new patient you will be asked to fill in a confidential medical questionnaire. It is important for your own safety that your dentist knows full details of your medical health and details of any drugs that you are taking as this can affect the dental treatment that you receive.

Following this initial appointment and likewise for existing patients of the practice you will be asked to update this medical history at regular intervals so that we are aware of any changes in your health and medication as they take place.

COMPLAINTS PROCEDURE
As well as being told when we do things well, we also need to know when things don’t go quite so well. When this happens, you can support us by letting us know what the problem is, as soon as possible. It is hoped that the relationship built up between you and our staff will enable you to raise any issues or concerns you may have.

Any formal complaint should be addressed to the Practice Manager who will initially respond within 3 working days. The complaint will then be investigated and a final outcome reported back within 10 working days.

Should patients not be satisfied with the result of our procedures, then a complaint may be referred to the Devon Primary Care Services Tel. No. 01392 205205. Or for private patients, the Dental Complaints Service Tel. No. 08456 120540

FAILURE TO ATTEND APPOINTMENTS
NHS Patients should be aware that if they fail to attend or cancel 2 appointments without the required 24 hours notice, their NHS eligibility with the practice will be lost and no future appointments made. This has been agreed with the Devon Primary Care Trust.

Private Patients may be charged for failing to attend their appointments.
